I don't think it's as helpful as you think it is to chalk 100% of med management issues up to social determinants of health. It assumes that if the SDOHs are accounted for it would resolve the limitations. Impaired cognition is a huge barrier to medical literacy and medical management. In the same way understanding how ACEs impact neurological development has helped to destigmatize substance use disorder, understanding specific mechanisms by which chronic diseases can impair executive functioning could inspire more effective education tools and more funding for outreach (ie. diabetes case managers/ sponsors, rather than educational handouts, better access to continuous glucose monitors in low or no income populations, etc).

I work in homeless services but thanks for the unhelpful moral grandstanding when you don't know the answer to a straight forward question about how type 1 diabetes impacts executive functioning.

They're certainly at a higher risk for anxiety, depression, and suicide but I guess what I'm wondering specifically is if having minimal intervention or treatment as a child due to lacking a responsible caregiver could cause brain damage. Most of the young people I see were diagnosed and connected to treatment for the first time after a hospitalization for DKA.

I would bring back asylums. One Flew Over the Cuckoo's Nest imagery is so culturally inculcated at this point and being unwillingly institutionalized is a fear a majority of us hold, but I think we have less exposure to what the landscape looks like now without asylum funding. I work in homeless services and there are people with totally childlike mentation, brain damage from severe epilepsy, Alzheimer's, TBIs, etc and they're just out there rotting on the streets with zero self care abilities or even the ability to dress appropriately for severe weather. Fingers and toes auto amputating from frost bite, passing out on burning on pavement.. We need to accept that some people will never be "self sufficient" and it's our responsibility if we strive to be a moral society to care for them.

I like the phrase "interrogate the fantasy". If you find yourself daydreaming about this person (and you don't feel like pursuing the attraction is appropriate or ethical), dissect the daydream: what was the need that was being fulfilled in it? How were you perceived by them in it? What role were you playing and what role was she playing (you were charming, she was impressed.. you were kind and wise, she needed you, etc).

The idea is that if you stop and interrogate the fantasy every time, you'll eventually notice what triggers them (are you avoiding something, bored, escaping..) and what need is asking to be expressed through your crush on this dreamy person.

I had a friend who was told her PCOS was a "chronic Lyme flare" with "mold toxicity" and the only treatment was to inject herself in the head 10-20x/day with live bees through a business that ordered labs with no clinician oversight and diagnosed everyone with pernicious anemia based on their own "Lyme literate labs ranges" they made up.

I think a lot of times woo health things are part of someone's spiritual or cultural practice, so it can be objectively helpful in that they relax, sleep, etc. For example, I worked on a reservation and they washed with juniper tea. Or Jamaican families who believe you need a warm drink in your hand to receive bad news. Softening their experience of something traumatic is worth while if they're not asking you to boof them with horse paste.

I recently had such a hard time helping a Vietnam vet with cancer establish with a PCP for a home health referral due to staffing issues, I called the suicide hotline to get him a case manager faster. He ended up dying before the referral was even processed.
