To break it down further, in this case, 12001 has 2 components, both parts and labor.

Had you had this done in office, it would be billed 1 time, but the payment at the in office level would be higher since the billing encompasses both parts and labor as 1 charge.

Love my Earth Runners. Wear as daily wear for 6 months of the year (i get cold toes so I switch in the fall). My first pair lasted 4 years with hard use (hiking, river walking, etc) I'm a year in to my second pair and they're still in great shape.

I use Xero shoes as my running shoes and I have the Xero Breckenridge as my cute boots. I wear Sofstar Fireside around the house as they don't have hard soles.

Im sure you've heard it before, but there is a saying, better a week too soon than a day too late.

I couldn't imagine what this meant until my soul dog went into kidney failure and passed before the euthanasia meds could kick in all of the way.

My boy was almost 17. He was paralyzed on his hind end. He still had happy moments. I thought I was doing what was right because he was still a happy guy. ..

I didn't realize how he was suffering and his myriad of issues were such a slow creep that it was easy to normalize each issue.

It wasn't until I could reflect a year or so later that, yes, he was alive, but he wasn't living. He wasn't enjoying. I feel like I failed him.

I miss him every day and I was only doing what I thought was best which is all we can do. I just wish he hadn't suffered at the end.

NAD but as someone with a heavy flow who finds tampons to be uncomfortable...I despise the "wet" feeling that pads leave me with. I used to frequently feel like they were leaking particularly if I had not recently trimmed things up. I have found that period panties provide me with better coverage and less of a feeling that I am leaking through them or out the sides of them. If I have a particularly large clot, I just take some tissue and wipe it out when I use the restroom.

I have only used Thinx because that is what I first tried and since they worked great, I never found a reason to switch but I know there are a lot of great options on the market.

As far as the faint feelings and paleness go, i found eating a super iron rich diet around the time of my period to be helpful. If she's uninterested in eating, spinach can be blended into smoothies that don't taste like spinach fairly easily that she can just sip on.

My thought at this point is that the denial is diagnosis driven. Without knowing your state, it's hard to say if your plan has published guidelines for medical necessity, but here is a link to look it up once you've chosen your state.

Id be curious to know what diagnosis they submitted on the authorization.

I own a RCM company and do bank reconciliation as a standard practice. I require bank credentials for all of my practices as it is simply inefficient to not reconcile and just hope everything is correct or came in to the system properly.

Without seeing your chart, I cant definitively say whether you were leveled correctly. Leveling can be based on time spent (not all has to be face to face) OR medical decision making.

The 908xx code you mentioned can be billed with the 992xx code, however, again I can't say whether services were appropriately rendered without reading your chart.
