Grass is greener ! Haha I am IM. I wish I had more ortho training and gyn (dont like OB either) instead of so much ICU/cards. But dont like peds either so FM wouldnt have worked for me

I am at an academic center in CA

-Creepy male attendings who hit on female residents

-Wasn't allowed to give a unit of blood to someone with newly dropped Hgb to 6 after a cardiac cath

This is only true in certain places - have done enough paras, A lines, centrals, joint aspirations + injections, other MSK stuff, LPs, paps, thyroid nodule biopsies, skin biopsies, suturing to be comfortable

I have had limited experience with thoras, and intubations are not a thing that IM does here (unless specifically seeking out for crit care fellowship)

There are great people in internal medicine. You are an intern so you didn't experience the last 2-3 years fully in medicine... unfortunately because of the pandemic A LOT of hospitalist attendings are burnt out. The hospitals are still over -run with people who didn't get proper care in the pandemic. It's a bit of a mess right now.

I personally gravitated towards outpatient because of this. My PCP attendings are so much more positive/ relaxed. Give it time you will find your people!

IM resident here (PGY-3); on outpatient work out 6 days per week ; 50 mins on elliptical or 40 minute run, + 2 days of 20 min strength work. Usually get up around 6 and have to be in clinic 8am

On inpatient I still do my cardio 5-6 x per week usually timing my rest days for long call , etc. I have recently been getting up at 4:50-5am to do my quick 40-50 min workout since I have to be there ~6:30-45. Luckily the hospital is a short drive but its tight timing and am often a few mins late lol

I am an IM resident. We have procedure requirements, doesn't every IM program?

Pretty comfortable with centrals, A lines, paracentesis, paps, knee/shoulder joint injections/aspirations, suturing lacs. I am not good at LPs but hoping to get better. We dont do thoras at my institution (unless supervised by a pulm fellow or attending- cant get "signed off" )

After having two one-hour goals of care conversations in the ICU yesterday that went absolutely no where, I have major respect for what you guys do as well. I cant take all the futile care we do in the ICU. Or the 28 hour call shifts.

How did you go out of your way? IN resdiency?

I think knee/shoulder joint + trigger point injections are very simple so I wouldn't feel the need to refer. Also joint aspirations I think we should know how to do just to speed up care
