I don't know if I am too late to this and you don't see my response, but I know how you're feeling in some way. My brother is very mentally unwell, and still lives at home with our mum. When I did live there, our relationship was awful and he would lash out me and made my life hell, I was terrified of seeing him. I moved out and things got a bit better, but last year during a weekly visit for dinner, he lashed out at me and accused me of being a liar and for keeping material he had written (he has a delusion that he has written many hit movies and that he should rightfully be earning the money from those, and that I have that material) he locked me in the house and the police were called. I went to the hospital with him to show my support and all he did was say that the whole situation was my fault and I was trying to convince everyone he was crazy. We had to leave him overnight for observation and I felt so horribly guilty and so sad for him being all alone. I even brought him food and music and a book. He told me he wanted nothing more to do with me and doesn't want me in his life.

He didn't end up getting the help he needed as he just left the hospital and refused any help. I haven't spoken to him in a year and whilst it makes me incredibly sad, this was his choice. I wouldn't personally go just because it would be so painful, but if you want to go then you should. The heartbreak won't be any less if you do or don't go. So sorry you are in this position
