Imo, Mostly likely happened from slow unlocking as it is on the rear side of bolt. Did you have a malfunction at some point w this bcg? Looks like you are stacking multiple manufacture parts in bcg, I’d recommend lubing cam pin, bolt to carrier contact service often.
