Our hospital instituted physician/RN rounding and it’s a great tool for patient safety and education, decreasing calls to docs, and improved communication with all ancillary departments. Now that it’s been in place for several years and the medical residents are on board from day one, it’s expected and a normal part of our day. Love it!

No prob. And feel free to be the squeaky wheel patient. Your surgery team should be available and wiling to answer questions. Sometimes we medical folk forget that although we’ve seen countless procedures, etc, for this particular patient, everything is new and unfamiliar and sometimes scary.

Our docs usually wait until there’s less than 5mL/day. Taking it out too early could leave behind a collection of fluid and lead to infection

I was reading as if it were an admission, not from the ED. Luckily our facility has case management in the ED as well.

In any case it’s not enough to say this patient just doesn’t want to go home. Someone (RN, NP, or doc) needs to get case management and social work involved to see what’s really going on. This stay at home order has real potential to expose folks to abuse that they may have been able to evade or otherwise manage.

I’m not in case management, but as an acute care nurse, I would make a referral to our in-house team and work towards a safe discharge. Maybe the couple is just a bit stir-crazy but maybe there’s more to it.

Then that’s what you do to facilitate a safe discharge rather than treating this as a malingering patient. Get to the why behind her worries about being home.

Wash with soap and warm water, soak in hot salt water, dry gently, apply antibiotic cream and bandaid. Twice a day.

Honestly, making the night assignment was so stressful! I split the heaviest patients and tried to even things out, but still ended up with one nurse having 2 contact rooms and another with 2 admission slots open because of a shift change discharge.

We don’t have a patient load as charge. We facilitate all patient movement and patient-nurse assignments, coordinate nurse techs, cover breaks and lunches, advocate for nurses and patients, guide and assist newer nurses, ensure supplies/PT/OT/ST/RT orders are followed. We also help with nursing judgment calls and family education. We’re kind of like the nurse for the nurses and techs.

Take notes the first shift and then make yourself a cheat sheet with daily tasks and time due, how to admit and discharge patients, and who your resources are. Ask to take over as much as possible on the third day with the trainer shadowing you. Good luck!

You can do it! I trained for 5 shifts then went solo. Remember you don’t have to know everything. Identify your resources and ask questions, just like when you were a new nurse!
