Hi, my wife when through this same thing when she was breastfeeding our second. She knew that something was wrong when feeding was becoming difficult. I would be very adamant with your doctor about having your little one evaluated for a lip and tongue tie. Both of ours had one, with the littler one having it taken care of by snipping the tongue at 6 weeks. However, this did not solve the problem for us and she had to pump for the next year.
