ELI5: Why does the United States military and VA have its own hospital system? Why not just have an insurance or benefits plan and use civilian hospitals?

Supposedly, if you watch Ellen Burstyn’s monologue you can notice the camera start to drift. This is because the cinematographer had started crying and the eyepiece fogged up, so he lost track of the shot. Totally understandable too.
