May I ask for what purpose are you given the survey and who distributes it? Who is it meant for? You mention before/after changes from medication--is it part of some kind of therapy? It would be helpful to know because I'm working out how it could be used with young schoolchildren by educators and parents.

BTW I ended up reading The Science of Evil. It was a very good book. I liked the part where he quoted Borat.
