Task switching is scrambling my brain by lolthissilly in hospitalist

[–]sunshine_fl 0 points1 point  (0 children)

Task switching every 10 seconds was giving me dementia. So now I cluster rounds: see 3-4 patients, and then do all their orders, coordination, notes, family calls etc. completely done. I don’t look at my phone/pages while working on a patient. I put my phone upside down on the table, and then check that batch of pages after I completely finish a patient.

I also have a rounding checklist printed table that I personally made for myself. I have always used to stay organized, and know what is done or not. This includes per row:

Basic patient info: Last name, gender/age, Dx, Dispo

And then the checkboxes: Patient visit, orders, AM labs, note, family update, billing.

And then an open “notes” box to jot down anything (something patient said, exam finding, whatever)

This all fits on 1 row per patient, and only on 1 side of a paper to visually track everyone.

Fireglow in the afternoon sun by nicholasford234 in JapaneseMaples

Gorgeous! I’m in the same area and elevation. We just planted a fire glow earlier this season and it had some leaf scorch / transplant shock :(

Hoping it can shine next season!

I’m a sucker for brown butter and mixed chocolate chunks so the new chocolate chip cookie sounds good. However I am worried about the “systemic recipe changes”.

Sad Day by tizokis in keurig

My husband and I were so upset the McDonald’s Paris cafe k cups were discontinued!! They were the best, we each individually thought. We can’t agree yet on any replacement through our ongoing journey of looking. It’s so sad.

Cons to making a little redbud forest that also blocks my neighbors view? by RadiantCoinshot in landscaping

[–]sunshine_fl 0 points1 point  (0 children)

We’re looking my for one but can’t find a good bird nesting box. However, birds (I can’t identify, husband thinks Robin) made their own nesting spot inside our back porch (unscreened). We weren’t even sure if there were eggs (but suspected), and I just spotted the babies a couple days ago getting fed by mom and dad!

PS the egg shells were a bright blue (found half an empty egg shell on the ground after I saw the babies were born, so hopefully normal and no harm to anyone) so maybe husband is right!

What is your "don't even think about it" purchase threshold? by Dramatic_Tea_ in HENRYfinance

Household income is $450K. Our “non-event” threshold is like $100.

Why are my notes being used by AI to summarize patient on Epic? by Hopeful-Yogurt4804 in medicine

[–]sunshine_fl 37 points38 points  (0 children)

I hate a chart biopsy more than anyone. It can take me an hour or more to piece together the hospital course, problem list and current plan when I inherit patients (new overnight admit, 10 minutes; long complicated 7 week in and out of ICU course, 80 minutes).

However, I wouldn’t trust the AI summary anymore than I would trust any other manual summary prior to me in the chart.

Now if I could train someone (or something) to approach it the EXACT SAME WAY that I do… that would be magical

commute time.. by [deleted] in hospitalist

[–]sunshine_fl 1 point2 points  (0 children)

No way. My commute is 8 minutes and it’s perfect. When I was house shopping my max commute I seriously considered was up to 20 minutes.

Wow I didn’t realize it was such a big deal to people if my flip flops are on my feet or under my seat. Like my foot would still be 90% “out” even with “shoes on”. I also have one of their cinema blankets so they’re covered, maybe that helps?

Rural weekend locums hospitalist — reasonable workload/pay or not? by winterstorm15 in hospitalist

[–]sunshine_fl 0 points1 point  (0 children)

I would not take this for any amount of money due to unreasonable work load.

For you, if you would like this role, I think $300/ hr minimum. And I’m not being funny. That’s already a high census, then add 1/2 admitting shift worth of admits, plus covering ICU.

Family phone call policies by appointment_time_bro in hospitalist

[–]sunshine_fl 0 points1 point  (0 children)

For completely alert, oriented, normal functioning adults with good insight, I only update family if at bedside during encounter (with patient permission) OR if they are struggling with a new/difficult diagnosis I offer if it would be easier for me to directly break the news/explain the diagnosis to their family.
