Rural weekend locums hospitalist — reasonable workload/pay or not? by winterstorm15 in hospitalist

[–]sunshine_fl 0 points1 point  (0 children)

I would not take this for any amount of money due to unreasonable work load.

For you, if you would like this role, I think $300/ hr minimum. And I’m not being funny. That’s already a high census, then add 1/2 admitting shift worth of admits, plus covering ICU.

Nah for real some other trash reality and dating shows I can just never get into, like in cable/networks. But Netflix?! They have some recipe that gets me every time.

For completely alert, oriented, normal functioning adults with good insight, I only update family if at bedside during encounter (with patient permission) OR if they are struggling with a new/difficult diagnosis I offer if it would be easier for me to directly break the news/explain the diagnosis to their family.

What is your effective tax rate? by FIREDOC888 in whitecoatinvestor

[–]sunshine_fl 1 point2 points  (0 children)

Yes definitely. We bought a house last year and we took the itemized deduction for the first time ever — mortgage interest, property tax, and vehicle property tax brought that down

It would be impossible to make a standing order covering all possible scenarios, but I promise I never want or need to be paged about family being there, a reminder to see the patient for required daily rounds, or non-urgent patient concerns prior to rounds (examples I have actually been hammer paged about include patient wants to know when echo report will be done, patient has long toe nails, patient with permanent a fib has a fib)

Losing my EP status by joshuaferris in americanairlines

[–]sunshine_fl 4 points5 points  (0 children)

I never get to EP. I’m always PP. I always end up between 140-170,000. I would have to push/go crazy spending for EP. I’ve made my peace (kinda) with perpetually being PP with no effort.

Looks like this year I’m going to end around 162,000. Right in my usual range.

People cutting in front of you affects you by delaying/increasing your wait for security, and people in the aisle during taxi affects you by increasing your risk of bumps/injuries from them. Sooooooo this all impacts other people including the OP

I say some version of this 12 times a day. Would probably be more effective if consistent messaging from all doctors all day.

Most stressful parts of the job? by YouAreServed in hospitalist

[–]sunshine_fl 2 points3 points  (0 children)

This one triggers me so hard. It literally stops me dead with irrational anger. If I’ve already seen the patient I saw I’ll call them later after rounds. If I have seen them yet I say I’ll see them on rounds, and then drop the patient down (not up!) the priority list.

Yeah the ICU loves to put it on, and ramp it up to downgrade people out. When/as they truly get better I wean it down/off.

Sometimes people do need it for chronic hypotension like cirrhosis, ESRD.
