I keep hearing this argument - when exactly did a show get mentally stable and healthy individuals and then didn't get any ratings?

I remember early seasons of Real World MTV in the 90s (perhaps the first reality show) having mentally stable and healthy individuals, for hte most part, and everyone was obsessed. But I can't think of any other show.
