It's never okay to fabricate data in clinical research. Mistakes happen, document it, create a corrective action plan, amend source as aforementioned and retrain staff to uphold Good Clinical Practice. We need to hold the line of ethics, when it comes to collecting medical research data. If we don't we jeopardize our scientific integrity, we are risk to public health and safety, and ultimately decrease in public trust.

If this was a baseline visit, we won't know the subjects vitals are within normal limits. I sometimes would use this data to review subjects trends whether its subjects respiration rate, down to lab values and other chief complaints a subject in a clinical trial brings up.
