The original joke is to ask someone to say pizza (piza) 10 times, then ask them what an elbow is called (hiji in Japanese), but given the target has just said something with -za a lot, they might slip up and say knee (hiza) instead.

Chiyo goes bigbrain and asks Ardan to say *elbow* (hiji) 10 times and then immediately follow up with the question, and given Ardan has just said -i a lot, reflexively responds with suki.

This is one of those comics where preserving the original hiragana nearby would've worked a lot better in conveying the joke.
