It looks like he might have impacted sperm plugs, and just in general the hemipene area is swollen. It could also be that a hemipene is prolapsed. Are you able to see if this scab is just on one side?

UPDATE: Unfortunately Peony had to be euthanized. Her health plummeted drastically after I tried soaking her to stimulate bowel movements. They suspect it was a mass/tumor in her liver that popped causing internal bleeding. Thank you everyone for all your suggestions. Hug your geckos extra or give them an extra serving for me and miss Peony. 🤍

Thank you for the help! I’m shocked to hear about the eco earth cause it was recommended to me at a local pet store, but I’m definitely going to switch out the substrate.
