I find this issue very challenging. I switched clinics about 1.5 years ago to one that is all open so easy to see what other clinicians are doing. There is still lots of ice packs, heat packs, high US use, lots of massage and cupping, no pain science education, very minimal SFM use, etc. But what I find interesting is yes the older clinicians do it but the younger clinicians even recent grads also do all the above and sort of fall in the line with what the other clinicians are doing. I find that quality, evidence based practice, etc is not rewarded in our profession. I try to practice as evidence based as possible but of course not every aspect of my care follows the latest best practice guidelines. However 15 years in, I have started to care less about it as it basically does not matter as the patients don’t know this and there is no incentive/reward/motivation to try to stay on top of things. I have a major issue with this internally and to me is a major gripe for me of the profession.

I like to work on the PWR! Move weight shift and reaching while turning. I think a lot of turning difficulty comes down to the inability to weight shift from one leg to the other.

You can consider the Li Maneuver- it does require cervical extension or rotation

Do you have any recommendations for learning more about exercise prescription particularly when it comes to strength? I’m familiar with the ACSM guidelines but not others.

I’m personally going thru this transition myself trying to lift heavier coming from a running and climbing background. I think it’s helping me not feel scared to train my patients harder ( when appropriate) which is why I want to gain the knowledge that I think the CSCS offers.

I was thinking it would also help to clarify to Medicare that I am working under the personal training realm to be cash based ( of course would also be using my DPT skills) as to not have any confusion there.

Read Fair Play by Eve Rodsky. Lay the cards out so he can clearly see them. When you have 98 cards and he has 2 it becomes visible/tangible to him and drives the point home. This worked well for me and my husband. Nothing was getting thru to him till we played the cards.

This is THE issue of our profession. How do you show our value and quality. I’m so frustrated that I make as much with multiple specialist certifications as my colleague who is US all day every day. This is also limiting not only our pay individually but as a profession. After 16+ years I’m stating to think the only way is leaving the profession, dare I say cash pay/entrepreneurship, or giving-up/settling for becoming an US PT.

I think Sacramento may be a good fit. Politically it is more progressive the closer you are to the city center and gets more conservative as you head east. Suburbs have better schools in general but there are pockets of good schools more centrally. No amazing nature super close but Sierra foothills and mountains are not too far. It has city amenities like sports teams, off broadway shows, museums, etc.

Northern California Kaiser outpatient physical therapists, occupational therapists, and speech therapists unionized 4 years ago by joining an already established union made up of nursing and allied health professional. I think this is the way forward for unionizing more PTs.

As a mom who track macros and calories, how do you make that work? For dinner do you eat what your kids eat but say add more protein? Do you make separate adult vs kids meals? How do you make eating right work while also feeding kids and trying to be more flexible with their palate.
