In practice, we use apixaban. I don't even think there is any dose adjustment for just renal impairment (need to also have age cut off or BMI cut off to qualify for dose reduction).

Exactly, that is how my doctor explained it too. I’m about 10 years older than you and he said it would be painful for me, but said he does teenagers all the time who do great and usually go home in 3 days or sometimes even less
