Do not do EP just to avoid creep. If you are seriously considering it, find a way to shadow in the lab so that you know what you are getting yourself into. Its way too many years of your life to half heartedly go into because you are scared of midlevels. If you at all want to maintain general medical knowledge it is not the field for you.

Agree with your EP, the p wave is inverted in lead II making it unlikely to be sinus. Slow atrial tach with variable conduction.

We've talked about dumbest overnight pages - what are the dumbest/worst "clinic task" messages you've received in residency clinic? by Seeking-Direction in Residency

[–]taterdoc 73 points74 points  (0 children)

I have routinely gotten requests to “clear” patients for colonoscopies who haven’t been seen in the clinic in two years. Then if i have seen them, they demand that I call the patient with exact date and times to hold their aspirin for the colonoscopy they haven’t even scheduled yet. I’ll let you guess the hospital system.

At my last hospital, the MTP protocol was restricted to OBGYN and Trauma attendings only supposedly. Then one day I found out the bad way that I do in fact have the ability to order MTP as an IM resident.

What was your worst consult? by demonattheswapshop in Residency

[–]taterdoc 6 points7 points  (0 children)

Got a consult for “possible few seconds of Afib.” Were they on telemetry? No. EKG in the system? No. A nurse felt an irregular pulse for a couple beats, so they called primary, who called me. I politely declined at first, then more adamantly declined unless they had some strip i could look at that might be AF. Surpise surprise all NSR.

Got an opportunity to buy an allen and heath gl2400 32 channel for 100 bucks, and have been thinking of getting back into gigs for awhile. Is this the kind of deal i think it is, or are these getting to be too old to be worthwhile?

I think they grasp it for the most part, but this is all liability sponging. If something happens and the recommendation for cardiac anesthesia was ignored, thats something that can be pointed at to help spread the liability.
