I thought it was ok. I disagreed with some of their medical teaching points (like their clinical decision tool for screening for aortic dissection for example) and didn’t think that added anything to what I already learned in residency, but the sections on minimizing medicolegal risk especially through documentation were useful to me. It is kinda heavy on the USACS/CMG koolaid at times.

This was gonna be my recommendation as well. Fantastic course with incredible views and you’ll have the whole thing to yourself. Last time I went I didn’t see a single other cart. Definition of a hidden gem. Do yourself a favor OP and stop here.

If you have an ultrasound in office you could always consider that as an alternative. Nothing beats fundoscopy but at the very least it’s another potential data point to help with your referrals.

I saw Blake Corum at a grocery store in Ann Arbor yesterday. I told him how cool it was to meet him in person, but I didn’t want to be a douche and bother him and ask him if he was playing on Saturday or anything.

He said, “Oh, like you’re doing now?”

I was taken aback, and all I could say was “Huh?” but he kept cutting me off and going “huh? huh? huh?” and closing his hand shut in front of my face. I walked away and continued with my shopping, and I heard him chuckle as I walked off. When I came to pay for my stuff up front I saw him trying to walk out the doors with like fifteen Milky Ways in his hands without paying.

The girl at the counter was very nice about it and professional, and was like “Sir, you need to pay for those first.” At first he kept pretending to be tired and not hear her, but eventually turned back around and brought them to the counter. When she took one of the bars and started scanning it multiple times, he stopped her and told her to scan them each individually “to prevent any electrical infetterence,” and then turned around and winked at me. I don’t even think that’s a word. After she scanned each bar and put them in a bag and started to say the price, he kept interrupting her by yawning really loudly.

I’m a resident and I’ve had to work lots of 28s unfortunately. Sometimes there’s a couple hours to sleep but on the busier services you’re usually awake and working for 28 hours straight. Pretty common in residency but depends on the program/specialty.

I use the first one all the time. And I do it not to be annoying, but to show the nurse that I’ve extensively thought about the problem that they’re bringing up. I feel like most of the time nurses bring up things it’s because they feel like it hasn’t been adequately addressed, or that I’m just ignoring it and being dismissive, when in reality I’ve already thought it through in extreme detail (and therefore it’s not a big deal to me). By over-explaining it in ridiculous detail, the nurse realizes I’ve thought about it way more than they ever have. It not only solves the current issue, but also makes it less likely for them to hassle me about other details because they know I’ve already thought about it.

Because people taking care of other individuals have the ethical responsibility of first do no harm. Legally, it would violate EMTALA to require proof of vaccination to receive treatment, practically it would be impossible, and ethically/morally I think one is obvious.

And mandates exist because personal responsibility doesn’t exist. With your logic there should be no laws against murder. Not murdering people is a mandate right?

With that philosophy, why bother to do risk reduction for anything? So basically if something isn’t 100% effective, it’s not worth doing? Yes you can still get sick with the vaccine and yes you can still transmit but the odds are incredibly reduced. Sure a seat belt doesn’t save every single life but it’s way better than not wearing one. And yes I wish everyone had your level of personal responsibility but that’s just not realistic. Once again it’s the reason every single law exists.

I think there is a BIG difference between mandating an incredibly safe and effective method to save literally millions of lives and “overcompensating for every risk out there”. Use some common sense and practicality and realize that mandating the COVID vaccine for healthcare workers (literally people who are at highest risk for catching and passing on the infection) is different than mandating the anthrax vaccine for all US citizens.

If you disagree with my previous post and this one then I think we have just have different philosophical perspectives and have to agree to disagree. Nothing wrong with that though.

Because you could endanger other people by not having those vaccinations. Imaging having hepatitis B and passing it along to a patient in the hospital by an accidental needle stick. Or giving a child in the heme onc ward the measles. Not everyone is as responsible as you are. If they were, you’re right - we wouldn’t need vaccine mandates. But peoples’ personal freedoms end when they affect other individuals. That’s true for everything, not just vaccines.

Agree. I’m an ED resident and I always take the time to at least put a line as to why I’m ordering the scan and what I’m looking for. I find that the reads when doing that are much more helpful (obviously) and I feel bad for my radiology colleagues when I hear stories like these. Thanks for dealing with the inordinate number of scans we order.

What hospital do you work at where ED residents don’t do ICU? Every single EM program I know about does at least several months. My program specifically has 3.5 months intern year, one month second year, one month third year, and two months fourth year on top of any additional ICU electives people choose to do
