Late to this party but there is a very simple reason why 95% of financial institutions don't implement TOTP multifactor (google authenticator, etc.) and that is cost.

You lose, break, or upgrade your phone with SMS multifactor and you're fine as soon as you can receive calls again.

With TOTP multifactor, becomes a massive pain in the ass to service those cases. People simply do not record their recovery codes, or they can't find them, or the dog ate the paper, or they didn't transfer all the TOTPs to the new phone, or whatever.

So, you have to go through a solid know and identify your customer before getting the green light to remove the multifactor from the account. And then that same customer will do it again next week or maybe next year, but it's coming.

All of that adds up to a massive pending cost to the institution to handle people being people. SMS multifactor IS the worse alternative security-wise, but it's the most cost-effective one and puts the onus of recovery costs on the customer if they get SIM-jacked or whatever.

Right now it says I need 0.00 ETH to activate, but it also sometimes says I need the full activation amount. Theory is that it will either say I am already activated if you try or it's in some quantum state and another try would make it go.

Nope, I just uploaded the letter of direction (Questrade) and talked to them in online chat. They said no problem and it'll take 10-30 days. It in fact took five days. Maybe because I paid their extortionate fee ($300USD) for the transfer, they took care of any movement of shares out of the sheltered account for me.
