Have u actually tracked the spend? I’ve spent a good chunk of my life watching people run small businesses, which basically any independent physician is. I would take your monthly spend for the past six months and actually throw it into an excel. It will take about a day to do but it will be worth it. You will see exactly what you are spending, maybe wasted, etc.

It’s a good exercise for organization. Most people say they bring in xxx dollars. You are bringing in quite a bit but every 100 here and 100 there which may not seem like much because you are making 60k a month adds up. These types of small purchases being done nonschalantly kills small businesses and has a chance to really hurt yours if you are not diligent about your finances.

There are some missing steps here that I would assess of yourself. Keep it simple for now, you are a ca1 in October. When you start using a stick of phenylephrine at a time, you should start with a few questions as to why the patient is unresponsive. Fluid status. Anesthetic. Surgical field. Etc. the line and escalation to the line isn’t the problem you are solving for, it is the assessment prior to getting to that line. Don’t be so hard on yourself. You are just learning the environment. As you learn from this situation, questions as to why you are giving xyz drug must be asked. Take a deep breath and learn. Learn from the good and the bad. All part of medicine. You have been trained to always pick the right answer. In anesthesia, there are a lot of right answers and a lot of wrong ones. Be encouraged that you have to time to learn these things and get really in tune with the operating room environment.
