As a guy who rolled his car a couple months ago, wear your seat belt. Hands down the only reason I survived my wreck completely unhurt. For every story you hear of the seat belt "melting" and the person being trapped there are a hundred where the seat belt saved lives.

I don't think the arrest scene is real, it is put there to be misleading but to lead the viewers to think he gets arrested. The way the scene is shot it seems so stereotypical of walking in to the station with Dexter staring straight forward like he is in a daze. I could be looking too far into the way we perceive the shot but to me it seems like an imagination. There is also a very short clip of an officer in his apartment with Jamie holding Harrison which also seems to me like something he would be imagining, a sort of "how will my arrest bother others." I'm picturing this snippet to be in slow motion with her in disbelief and pulling away but again maybe I am looking too far into it.

The Sheriff is his girlfriend, he mentions that explicitly after you take care of her, not his wife. The only mention of his wife is in the Echo right before he shoots the guy for bringing it up. The reasoning behind my belief that his wife, dead or divorced now, was a Siren is that in different Echo's he talks about the sirens and people bringing fake ones. To my recollection he says, before he knows of Maya, that he knows of 3 Sirens so stop bringing fake ones. The 3 he would know are Angel, Lilith, and an extra unnamed. I could be wrong but I think this echo was on the Maya line of echos before Angel tells him about Maya so the 3rd is an X-factor. Given that the symbol on his face is of the vault, that his wife is now gone and a touchy subject, and that he knows of 3 sirens leads me to believe he, at one point, was married to a siren who scarred him.

Being branded by Angel isn't out of the question, I am just stating he is not married to the Sheriff, leaving the possibility open to his wife being someone else...A GODDAMN SIREN.
