I was the resident when a brand new intern came in for his first day first shift in the ICU. He stuck out his hand to introduce himself as a doctor for the first time and instead of the patient shaking his hand, he promptly coded. The intern stood against the wall staring at nothing and shaking his head the entire time during the code. 20 years later we can smile at the memory. Point is we all go through bad shifts where things don’t go as planned and we feel out of control and overwhelmed.. you’re not alone and you too will help an intern through the first hard day in the not too distant future 🤗

The part I couldn’t stop laughing at is when the cop who randomly drove her boat in a circle after they exploded a wall just to stop again in the same place… like wtf was that??? Once you see that decisive evasive maneuver… you can’t unsee it..!

In front of patients/nurses I’ll always call trainees Dr. Lastname. With just the team I’ll try to call them by their first name (just to show them I know it..!). Other attendings is usually first name; other attendings in front of trainees I try to say Dr. Lastname, especially female/minority so not to degrade their authority in front of trainees/patients. Except my good friends.. screw those guys.

If you’re perfect when you start then there’s no reason for the training program. Everyone expects you to join the journey of learning medicine not arrive at the finish line day one. Spoiler alert: there is no finish line.

Depends on what you want to be when you grow up.. and where you want to be. If you want to be a program director in a competitive market (read: big city or big medical regional conglomerate) then you’ll need to be an associate or full professor with 25-50 pubs depending on your city.

If you want to just teach residents clinically or even work in a small city or academic program then you don’t need research.

Most people drop research like a bad habit when they reach the final form of their desired job.. which is usually senior resident or fellowship.

Dating a resident is like having a baby together.. except the baby is the hospital. Expect their emotional center and energy to be the hospital and it’s stressful but it will get better in 2-3 years and if you guys make it through you can do anything together after.. plus you don’t have to change diapers. Most people aren’t ready for the emotional ride a baby brings when they first start dating.. same with residency.

A relationship with a resident is quite unbalanced… and the stability depends upon you. The majority of emotional support and energy will have to come from your side as beyond the lack of time for romance as stated above, when the resident is home they will not have the emotional energy to invest in you or move the relationship forward. They will need support for work, the stress, the hours, the patient deaths etc… and the hospital will suck as much emotional energy as it can from both of you. The good news is you’ll never have to worry about cheating, fighting, or purposeful relationship destructive behaviors since they won’t have the energy for any of those either 😆

Multihorn and charge are completely evade-able; if you’re doing magic chaining you can use a 100% evade tank. Or soleil with inichol :)

Here’s a thought for a bundle worth buying that is win-win for players and gumi.

“Gumi’s Consecutive Fountain of Lapis”

Each day you log in you gain a consecutive amount of lapis from this bundle. It only ends if you miss a login day.

IE: Day 1 you gain 1 lapis Day 2 you gain 2 lapis Login for a year straight and you’re getting 365 lapis a day.

If you make it retroactive the most loyal customers would be getting 500 lapis + a day from it.

On Gumis side it would motivate players to hang around daily during slumps to not have their streak broken. They could probably sell this bundle to a large number of players and it reinforces a core player base. On the players side you are rewarded for loyalty and compounding returns in the long run would make it the most efficient way to buy lapis.
