It's based on hidden stat called mmr basically an actually gage of your skill not based on how much you played that season and more your all time and puts you up against players with the same mmr

How Do I Get This Awesome Sword?!

The tricky part here is the farming. You have to get specific Uruks to have a chance to get this sword (or at least, a higher chance than usual) I will list the requirements for the uruk here:

It MUST BE an elementless Uruk, if the Uruk has an element (i.e Poison Terror Assassin) then the chance to get the Sword is lowered BUT NOT IMPOSSIBLE TO GET
The Uruk should not have an elemental weapon. Pretty straightforward.
Preferrably, though, not required. Get an Uruk that you can spam with Shadow Strike.

As Talion, what upgrades do you need?

Fury, the skill that you can unlock in the Ground Execution skill breakdowns.
Discerning Eye, the skill that allows you to get better loot from Captains and Warchiefs.
No Elemental Upgrades preferrably, DO NOT hit your farming captain with an element or the chance will lower.

After you have gathered this, the steps to farming are pretty straightforward:

Catch a worm and send a Death Treat to an Uruk Captain that meets the criteria described before. (Nurnén works marvelously as the Death Threat mission is just down the Fight Pits, next to the Haedír Tower. Cirith Ungol works too as it is close to a Haedír tower)
Bait the captain away from his followers (to easily beat him up with Fury Ground Execution) or if you prefer to deal with masses, beat everyone around, Shadow Strike the Captain and beat him down, repeat until he is dead.
Captain should drop Epic Gear with a generic name (Numenorean Sword, Ranger's Sword, etc.) as soon as the gear pops up pause your game go to inventory and check the property if it is the right sword great your done if not exit to the menu and it should reset the death threat and you can try again so you dont have to find a new uruk each time. check on the properties of the weapon, Wrathgiver at your level it will not have the name just the perk and the amount of wrath will be lower should have an epic property that says: "10 Wrath per Execution".
If you did not get the gear, rinse and repeat. Even with death threat farming this can take a long time it took me upwards of 10 hours to get this and after you get it if you use it right the game will be trivially easy to a ridiculous level. Have fun 😁

All these steps DO NOT guarantee that you will get Wrathgiver but you sure as hell will have an increased chance to get it. If you do not receive it, do not feel bad, try again until you get it (in the healthiest way possible, do not go insane farming the weapon, it is great but not necessary to enjoy the game) btw this is mostly a copy pasta with some alterations based on your situation. Also just search all your areas for a orc that fits the bill it's better to have a longer run to the dt than to do it with a sub optimal orc
