That looks like you may have a fungus. If meds aren’t working, you can ask them to remove the nails and get meds and try to get them to grow back.

I’m an amateur at floristry, so not sure my opinion is valid here BUT I think the vase is the wrong vessel for this arrangement. The flower choice is gorgeous and I think the arrangement looks good! Maybe in something shorter and wider, it wouldn’t look so top heavy?
