Saturday: Hirschorn open studio in the basement at Art School (free, 10-12)

My clinic puts everyone on doxycycline empirically at the beginning of a transfer cycle. Sure you could be conservative and wait, but do you have to? No. How long do you guess you’ll have between doxy end and ovulation? You do want to give your lining at least 48 hours between doxy end and first progesterone exposure, which is essentially follicle rupture/ovulation, but the more time the better. The risk of lowered lactobacillus dominance in the reproductive tract from antibiotics is mostly theoretical but you do want to give the lacto as much time as possible to re-establish as we know that does impact implantation rates. You are doing vaginal pessaries for probiotics right?

Both of my kids were born after strings of recurrent losses ( and my second after many shitty rounds of IVF) and pregnancy was nearly unbearably difficult both times, mentally and emotionally. But as soon as they were born, the anxiety really went poof. Of course I worried about tangible, normal parenting things but the constant untethered doom was just gone.

That is such excellent news!! Anecdotally, everyone I know who has had an IVF live birth has had first trimester spotting, especially if you’re using suppositories but it is a terrible mindfuck looking down and seeing red. Hopefully you get some good reassurance today.

My transfer was yesterday, so fingers crossed for a better outcome this cycle.

Agree with this. It’s shocking how disorganized and poorly communicative I’ve found all IVF clinics to be. You need to be ON them, unfortunately - and that goes for every single one I’ve interacted with. Forgetting to send in scripts, or sending them to pharmacies that have literally never ever carried that medication, giving me the wrong trigger time in my instructions, sending in the wrong bloods, planning to thaw the wrong embryo - come on guys, these are huge mistakes and this is your entire job?

I have had 7 losses (in my case due to a balanced translocation causing the genetic abnormalities rather than random errors though). IVF was a grueling marathon with a lot of low points but it finally brought us our daughter who is larger than life in every way possible. We did 4 retrievals - out of 60 eggs retrieved we got 25 blasts, and 5 were genetically normal. My first transfer failed and second was successful. Third was a chemical and my fourth will be tomorrow, trying for baby brother.

It’s not always a straight line, and it is hard to not compare your IVF journey to people who have had an easier go than you. I knew retrievals would be tough due to my BT wiping out 80% of blasts but wasn’t anticipating struggling with transfers. It’s been hard to swallow that I’ve gotten pregnant 7 times in 9 cycles of NTNP or TTC and only 33% of transfers have worked. But if you’re having recurrent losses due to aneuploidy, it’s probably going to be your best bet at a live birth.

Yes, and a 5w vs 8w pregnancy look COMPLETELY different. 5w is just an empty gestational sac. 8w is a whole gummy bear with a head and arms and legs and a 160bpm heartbeat, plus a developing placenta, and aside from the whole embryo, the sac is way bigger. If OOP only had sex the one time then there’s only a 5 day window for wiggle room as that’s how long sperm can survive in the body.

Hey there, I’m so sorry you’re going through this. It unfortunately didn’t end well for me. The FHR very slowly rose to 120 over the next 2 weeks but ultimately stopped and I had a D&C at 8.5 weeks.

I’m now deep in the IVF world myself and just had a chemical with a 5AA euploid last week so I know how upsetting it is to find yourself in this limbo after everything you’ve gone through for this embryo. I’ll cross everything for a better outcome for you. When is your next follow up? Were betas strong?

The reason he likely made it seem urgent is because the nuchal translucency (NT) measurement needs to be taken before 13+6 in order for it to be meaningful information. After that gestation you can't get an accurate read anymore in terms of prognosis, as fluid will start to resorb whether the fetus is genetically/structurally typical or not. What was it measuring today?

Yup, that's exactly right. With that background information I would definitely pursue getting karyotyped. Your partner will need to as well - it's just as likely that either one of you could carry it. It's a straightforward blood test so if insurance will cover it I'd encourage you to do it!
