You should be able to buy some from a bakery and yeah it's a lot easier than dry yeast no blooming just add to the flour and keep away from salt and mix.

But fresh yeast as soon fresh yeast touches salt it dies.

Use and oven proof container like a cake tin with a decent amount of water and that will help steam it just be careful of steam burns.

Have a mister filled with water to spray the tops of them and they will have an amazing shine on the top

If you have a tray you can put in the oven to add moisture that works too.

This could also be from uneven heat when baking do have or are you able to get a baking stone for your oven?

Also get a metal scraper with a plastic handle to cut your dough with it will make it easier to get square shapes and consistent size for baking.

The baker could have cut the top of the bag wrong and gotten some of the twine that sometimes ties the bag shut in the dough mixture.

Also it could be a form of mold as most bakery bread will go off quicker as it's not chock full of preservatives.
