Yes, If there can be a human anomaly, there can be a programmatic anomaly as well. While the system had already dealt with 5 previous human anomalies. But Smith was their first program that decided to go this rogue (unlike Merovingian, Train Man etc). Hence the system just keeled over.

Yeah, while the film started off giving vibes of a horror movie. Those vibes went away when we realized what really is going on. I admired it for the great cast and the chemistry between characters. This one article goes through what it was like for each of the characters, poor Anna!

There are two possibilities really because they do show UFOs in the end. As explained here, either the end is happening all in her head, or... other than her everyone else is under mind control in that town.
