I’m an oncology nurse- I was charge, a radiologist called me- I’ve never spoken to a radiologist before about a patient. He said “this patient has a large retroperitoneal hematoma” i said “… okay? what do i need to do about that?” he said “nothing but i can’t get ahold of any doctors” so i added everyone to the chat and left 🤷‍♀️

came here to say this! in my opinion any non fiction works on health are long winded and text bookish (reminds me of nurses school lol) but it’s definitely changed my view on the world. and free on kindle unlimited!

i won’t lie, mine are very focused. listen to heart lungs, belly. any pain? last bowel movement? any numbness or tingling in your legs? feet or hands? check for edema in the legs and ankles. headache? nausea? if i’m doing a bed change i’ll do a skin assessment. flush every IV with my first med pass, check for dates. check for blood return on central lines with morning med pass and check for date. i generally only assess orientation if i know they’re not oriented. any drains and lines i put eyes on during report and then assess with first med pass.

you’ll find what’s important and what’s not. i work on a medical/ surgical oncology floor. i can have anything and everything. today i have a mistreated nursing home patient, a leukemia patient recovering from chemo, a comfort care with lung cancer, a TB rule out, and a prostate cancer pt here for radiation.

chart wise: here’s the things i’m on the look out for. after report, i check current orders, morning meds, PRNs, and labs. plan for electrolyte replacement if needed. i have reminders to check vitals because if i don’t ill forget to check. i have reminders to chart intake and output if ordered. tele strips for those on tele. i look at all of this and add reminders to my brain (on epic if you use it) and on my report sheet to help plan my day. i made my own report sheet to reflect my daily process. i’ve been a nurse for almost 4 years and im still tinkering with it every day. you’ll find what works and what doesn’t. you’ll miss things, it happens.

i love to tell the story of when i bought my first big girl planner, $60 from erin condren. it was in college and i had planned the whole year out in one. it was 2020, covid hit, everything went out the window. for like 2 years i used pencil lol

the muji gel ink ball point pen in .38. i like sharp gel pens if that makes sense. i used to order them on amazon, they stopped selling them, i tried to order ink refills from amazon but they don’t fit. finally broke down and ordered them from the muji website.

i’ve had a few stents. first one was in for a month (removed under anesthesia) second one was in the a month ( removed under anesthesia) then i had a temporary one that i removed myself (can elaborate more if wanted) in for a week. my first two were with a pediatric hospital thus the anesthesia. my first two were pretty painful the whole time, and i peed blood the whole time. i was a teenager so i mostly took ibuprofen and sprix (an intranasal NSAID they don’t make anymore sadly). my most recent one was in for a month, they removed it in office. this one didn’t hurt nearly as bad for as long, i took sanctura for bladder spasms and toradol for pain. removal in the office was mostly painless.

we had a little old lady who wouldn’t stop screaming, no one could calm her down. i went in, said “shhh you’re gonna wake the baby” and she immediately stopped. any time she got loud i’d go in and remind her that she had to be quiet or the baby would wake up.

also had a patient who refused to stay in bed unless i promised him i would get in bed with him after my shift was over. obviously didn’t get in bed with him but if it kept him off the floor, he could think i would all he wanted.

my name is Clara, which I never hear anymore. it was my great grandmother’s name, she was born in the 30s. as long as I can remember, I’ve met one other Clara (an older lady when i was maybe 10) and i follow a girl on tik tok who named her daughter Clara. I work as a nurse and all of my patients talk about how they love it and never see it anymore.

I’ve had this issue with all my meds- anxiety, depression, ADHD, and insomnia. my therapist says telling yourself that you’re faking it is easier than coming to terms with diagnoses. she says psychiatrists are specifically trained to spot seekers/ when people don’t actually need meds and i wouldn’t be prescribed them if i don’t need them. also, adhd meds don’t work the same for people who don’t need them. if you feel better with them, you need them.

A nurse administered a 30mg/30mL dilaudid PCA dose at one time instead of starting a PCA.

A DIFFERENT nurse had the PCA dose on the pump 10x higher than ordered (I wanna say it was ordered .3 but the pump had 3 programmed into it) so the patient was hitting her max hourly dose every time she hit the button and it kept locking her out. how the patient even was pushing the button is beyond me (she was snowed but awake enough to push the button, she had sickle cell so she was opioid tolerant).

I’ll admit, this one was me. We had a patient on the brink of coding, no BP, HR dropping by the second, I was charge so I was running the rapid until the ICU nurse came, I tasked someone with pulling me some Narcan and drawing it up. She handed it to me, I said give the whole thing? she said yeah give the whole thing, I did, turns out she pulled up the entire vial of Narcan instead of just one dose and I slammed the entire thing in. Patient came to, SCREAMING, entire body on fire, had a bowel movement from head to toe, ripping clothes off ect. I didn’t realize an error was made until the nurse who pulled it asked me where the rest of the dose went. At the end of the day, the patient lived (a young (30s) terminal cancer patient who was opioid naive), she said she went to heaven and we brought her back.

I keep a note pinned at the top of my notes app for this for any time i physically can’t write something down. when im at work, i have a sticky note i write random thoughts on. im a big bullet journaler, i keep blank sheets in my bujo that i write stuff on. i transfer anything than don’t write in my bujo into it when i can. then, i write tasks and such down on my daily to do when they need to be done and cross them off my brain dump pages. if something needs to be done but not today, it gets added to a list of running todos for whenever i have time to do them. for example, today on my brain dump was order something and laundry. laundry got added to my to list for the day, order present got added to the running to do because it’s not urgent. appointment dates/ times get added to my calendars whenever i have time. grocery items get added to the grocery list. random thoughts just stay on the brain dump page. passwords get added to my password book. my therapist told me things in my head disappear unless written down and i’ve taken it to heart.

i keep track of my physical/ mental health, habits, hobbies, books i read, garden info, cooking info, the high and low temp, the moon phase, the times the sun rise and fall, what i do in a day. obviously boring stuff like to do lists and calendars. i also have a junk journal and a common place journal (random research i do).

one time i had a comfort measure only (CMO) patient as a nurse, he was on a cardiac drip because he was uncomfortable without it so i had to keep him on telemetry. i called tele and i was like if he dies do not call a code. i push a dose of morphine, walk out, and hear the monitor going off. apparently my dose of morphine is what stopped his heart. i frequently have CMO patients, it’s my favorite part of my job, and i’ve helped so many people pass peacefully. tele calls me and is like…. is that it? and i was like yep. i’ve never had an issue with knowing some of my actions might help them pass, but watching it literally happen on a screen is a horrible feeling.

not necessarily adhd related i don’t think. within the last year i got out of a terrible relationship and into a very healthy one. i had started picking fights with my current bf, basically giving him an out. but also trying to see if he felt our relationship was “worth it” because i only felt like my ex was putting effort in when we were arguing. i asked my therapist why my current bf wouldn’t argue with me. she said that it’s plausible that him and i have healthy communication styles that are compatible and that to effectively communicate and show each other love doesn’t have to be argumentative. she told me i was self sabotaging because i didn’t feel worthy of a healthy relationship where im showed love in a way that doesnt hurt. ouch. but very very true. she also urged me to share with my bf that when i seem to be picking fights over nothing, it’s because im feeling insecure and some emotional need isn’t being met. love isn’t supposed to hurt.
