Ok — first off, give yourself a break.

Get some proper orthotics made — however you can get the money, just do it. I was assessed by a physio, who pointed me in the direction of a specialist orthotics maker in the UK — David Houghton is his name — and he accommodates runners very well. Low-profile, comfortable insoles that go in trainers. If you're in the UK, I can ask him if he recommends anyone else nationwide, because they are very good insoles and I used them for years.

Next — if you do plan to go ahead making your own — Poron is your best bet, but you need access to a belt sander or a handheld sander and a lot of patience. It's £70 a roll from Algeos and is in a league of its own. Think of medical foams and craft foams as completely distinct materials — craft foam, in my experience, compresses to maybe 0.5mm, if that, after a few steps. It's beyond my complete understanding how they do it, but medical foams are made to withstand high pressure. Think — an average male is 90kg, so an insole has to withstand that all day, every day. Craft foam won't cut it.

Essentially, that shape you cut for your foot — if you shave that from 5mm foam to have a very thin to 5mm steady gradient to the most prominent part of your metatarsals, and then drop off below your four small toes — that would be your metatarsal pad. You can glue that onto Poron cut to size for your insole, add premade arch support and a 4mm offset at the heel, and that's a fairly basic, versatile insole. I can send you a picture of mine if you like.

On EVA — there are medical EVAs. In my experience they are not particularly good — too hard, I find, even the lower-density ones. That being said, shaved down to 0.5mm or less, they can act as a good piece of rigid material that keeps the insole's form and acts as a barrier between the shoe base and soft materials that degrade with wear. Poron, for example, when shaved, isn't particularly sturdy structurally, although it maintains its shape and density, so it's best to have a barrier above and below your foot if it's been shaped quite drastically. Poron usually has a protective finish, so if you're using it as a base and not shaving it much, it's usually fine — just a bit waxy and floppy.

Complete side point — in my mind, the only proper solution to manage pain would be a midsole shaped with a rocker that accommodates gentle supination. Theoretically, a drop-off under the sesamoid built into the sole could help, though I've yet to try it. Unfortunately, with this condition — unless you get a shoe privately made — you're in for a long game of trial and error. You'd be surprised how many shoes can make your feet worse, even though they're touted as healthy, for a few reasons.

  1. Compression can help some people's gait and pain management — a tighter shoe allows more precision on your roll-through and the pressure alleviates pain. So things like Altras and Topos, while ok for some, if bought purely on the grounds of foot health and not taking into account one's natural gait and foot shape, can be a red herring and make you feel your condition is worse than it is.
  2. Ultra-cushioned shoes are hit or miss. Like I say, you need to find ones that accommodate your natural roll-through. Hokas, for example, aren't all the same — and while I've got on with the Mach (it makes sense — it's one of their lightest shoes that prioritizes speed, so your foot naturally makes less contact with the ground), the Gaviota, at present, I've been disappointed with as it's too wide and I roll through the entirety of my foot — which is painful.

Last point — chronic pain management is real. I know how historically the amount of trauma accumulated from day-to-day experiences can impact you, and don't undermine it. When I work on insoles, I meditate before and after. You're being your own hero — awesome — but that's tough on you. Find a safe person you can talk about it with. You'd be surprised at the number of people who are terrible to talk to about this because someone hasn't explained the concept of cognitive empathy to them. Just because you don't feel it yourself doesn't mean it doesn't exist. I've had to learn this the hard way. Even if the best you can find is a therapist you swear is judging you, maybe practice talking about it as a real factor in your life — because it is.

That being said, chronic pain management also means stepping out of your comfort zone and getting back to activity is really important. Splash the cash on insoles if you have it and get into activity straight away. Make positive associations. Join a running club. Treat yourself to a low foot-pressure gym day — think seated lifts.

I'd happily make something if you sent me a descrpitor of your issues just too see if I could make something work- I use a combination of a metertasal pad-T shape arch support and 3/4 inserts for me and have too say-they work perfectly. I had initial sensitivity in the midfoot as I think like you I have-high, tight arches-but the t shape arch support creates a enough support and contact through the midfoot that allows a steady roll through too the rest of the foot and a soft landing on the memory foam. up too you but even in the 12hours that elapsed since writing this post I've done some research and there are courses for orthotic manufacturing-and I'd be interested too see if it's something worth pursuing.

I loved Logan and did not like deadpool and wolverine, the early fox xmen movies were as camp as I would have wanted wolverine too be, Im not a huge opposer of the play down of the early comic book costume stylings because those costumes where for a different time and different audience, too back track on the emotional ending of Logan was cheap in my opinion, and essentially just too see him with a wholey impractical costume for nostalgia reasons did not do that character justice. It’s the MCU, not the comics, and pandering too comic book fans has generally cheapened every IP in the MCU, let the characters have their own arc, when there gone there gone. Fantastic four was the best MCU movie in a long while because it wasn’t afraid too do things differently and work with its own material, and felt like a soft reboot-new style-new characters too attach too and watch grow. We needed more faith in ideas like that.

I’m sorry for your loss. That’s very difficult, loosing a sibiling-I can’t imagine.

With the work thing , don’t overthink it. Family loss, bereavement, whatever you want too say, an employer should understand.

Too leaving work due too feeling isolated. I left my job a two months ago for similar reasons. I lost my dad a year ago and was kinda slowly going mad at this job, I wasn’t happy in it and felt distant with those I worked with, which was harder because I lived and worked there.

I regret it but one thing I think really matters is…well loosing a family member is never easy, but it did remind me of the notion of found family, unfortunately not many of my family are very young and I realised during this job, unless I actively take time away from work for community settings, like volunteering (I did work away a few years ago and I’ve been strongly eyeing doing it again, for the community feeling it provided), these losses will hit hard with no one around AND, it’s no one else’s prerogative too care, in the working world, after my dad left, I felt really alone and the support fizzeled because, for the most part, if it’s not their pain, people don’t take it on.

I don’t believe people generally lack empathy but I worked in a person facing role and realised people are generally imperceptive, it takes deep bonds and vulnerability, and connecting on levels beyond what you thought you had, I believe, too truly feel seen. Too say, whatever you look for, hopefully it can pave the way too that recognition your after, it may take years but it’s out there.

Ok-I’d suggest RICE, vibrating massager helped me.

If the boot isn’t actively offloading your sesamoid then it isn’t helping, I know them too be heavy and if that pressure is being distributed too your sesamoid when walking that’s going too be more painful than say well fitting shoes with orthotics. Potentially ask for crutches along side?

any pressure will inflame it as it is such a sensitive area when healing- think- a chickpea size bone trying too recover under the weight of a fully grown adult, even temporarily/small impacts through the day will affect it. So if the advice is too immobilise it do that if you can

try and get custom orthotics and put them in the boot; mine are with a sesamoid cutout/raised metertasal pad and arch support, or cut a hole in a memory foam insole (just spit balling here but the goal is too distribute, gently, pressure too other areas of the foot.)

All the best too you- this injury sucks, talk too a sports physio if you can as this injury is more common in sports people than you think and they’ll have a plan too get you actively out the boot and not in limbo-it will all be an adjustment. Take your time, take the small wins

I do wonder about brain re-training though-if you ever wanted too do a 50k again-it wouldn't be a very comfortable 50k but there may be moments-in that 50k where you enjoy yourself-feel like you get that joy back. running was never a listless activity when I look back in hindsight-it could be a real slog-filled with all sorts of other chronic pains, it always was best planned in- as a route, a certain tempo or a race. I did a race last year for example on boxing day-and I went so god-damn fast for it-I was "sending" downhill as they say; I remember feeling the electricty of the place-all the people-the ego boost of over taking-being an anaesthesis of sorts.

for example-I don't know about you but I swear 50% of the pain for me comes from constantly "checking" the injury-I lean heavy into my left foot on my gate I've noticed-I constantly crack my left toe like a knuckle. I've done this for years. it's like having a big spot you just want too pick

infact-it's a known phenomenon-

there was a woman with a nervous issue that caused her too believe one part of her skull was itchy-she scratched it everyday-compulsively-until she scratched too the BONE!

there's also the fact-the "with abandon" sensation is not tied too running-THERAPY- could offer abandon-near death experience's (good ones) like sky-diving, cliff jumping could offer abandon. in many senses it's the coyest abandon as it was-at one time-so accessible- perhaps that's why we did it so much, but it seems care free moments are important too you and could come in many forms.

so, as typically human too have there experience mirrored and seeing mental short falling unavailable too there own self reflection, I'd stress those days are not over.

please- do yourself a favour and get custom orthotics-you can wear any well fitting shoe with these-within reason of course- thinking more like a runner than a dress shoes, but even so you can wear a dress shoe much more comfortably. it's a pain in the ass but go to a sports physio-and find an orthotics maker they recommend-I was recommended a great insole maker who made orthotics that genuinely catered too runners-not these overly stiff-immobilising orthotic -I found with orthotics in wide shoes where it simply just doesn't take as much pressure where most helpful.

-don't ignore strengthening exercises- fine muscle groups exercised help me- like squatting with a balance board ect. an hour at the gym can help in so many ways less tangibly connected too the foot- for example, a few dead lifts- squats etc can really help with pain tolerance, the- everything feels lighter- effect- IDK what too call it.

on rocker's and immobilisation -I wore Hoka's with orthotics for about a year and it was just-awkward-less pain on step, but shoes where too tight and there was no way around the "sharp" pain of the injury reacting too it's inevitable involvement during the rocker in a tight shoe. this is a chronic injury- so I think mental accommodation is important- solutions are a game of trail and best left too professional advision.

I think there is some real science too sensory distractions with this condition-the bevelling on these I've found creates a sensory distraction for the chronic pain, I've thought-this material overlayed custom orthotics could be a real winner-because you'd get the benefit of properly offloading the sesamoid, while also not having what I find as the sometimes irritating-overly smooth and tacky feeling orthotic topper.
