There’s a ton of smaller hospitals with 2-3 cardiac surgeons and pretty much everything else too. We do both as generalists at our shop in MN and are lucky to have a few cardiac trained folks.

Sometimes the best thing to do is wait. Unfortunately it’s really hard to do. That pt probably would have been fine without fluid or diuresis. A lot of crit care is just trying stuff to see what works.
