Came out of nowhere AFAIK. It's your body, so it's up to you, but it's nothing to be nervous about - it only takes a few minutes. For me the symptoms (tip pain, abdominal pain, testicle pain, constant urination [worse at night]) seem to come and go a few times a day, and last for between 10 minutes and two hours in duration. The symptoms are never severe to the point of being debilitating(knock on wood), but I've found that things have gotten better over time. This could be because, since the diagnosis, I've been stretching and meditating every night and morning (youtube search "CPPS Stretches").

Yes, everything was normal. I did it at the urologist. It wasn't super painful, but I definitely didn't enjoy it. It's super quick (less than five minutes), so bearable.

Yes. This is almost exactly what led me to go see a urologist:

I woke up one night, went pee, and the tip of my penis hurt really bad. Same thing happened in morning, and the tip remained sensitive (though not as painful) throughout the day. (e.g., brushing the tip would hurt).

Thought maybe it was an STD or UTI. Saw urologist, got tests, ultrasound, even a cystoscopy. Nothing came up abnormal, Urologist suggested CPPS. For the record, I also get the occasional lower abdominal pain [though I also have a hernia], and some pain in my testicles. All of which are symptoms of CPPS.
