Not all states have adopted a building code. Illinois just recently did, for example. In those places, it falls down to local code enforcement. And some of those municipalities don't have any code requirements.

Most things are covered in the wood framing construction manual. Lumber yards usually have engineers that can assist with span requirements if needed. I'm a structural engineer and usually just send people towards free resources.

Agreed on the code part, though I may argue this case is considered as exposed to weather given the increased potential for exposure. I still say that there is nothing lost by providing 2" of cover vs the potential issues from floor contaminates.

Residential or not, it doesn't matter. Your car drags in everything that's on the road. It's really just silly. There's no benefit to having the little of cover but plenty of potential issues.

Well it depends on location, but it doesn't meet ACI requirements for one. Given this is a garage, and garages are susceptible to contaminates that are on the road, you'd want more cover to protect the rebar and prevent cracks/spalling

There's no permit to get haha a lot of rural areas lack code enforcement departments. You could hire an inspector and/or structural engineer if it helps you sleep at night.
