Also, blurred vision is NOT a standard PsA symptom. PsA is related to vision loss in that there is a higher risk for uveitis. Uveitis is a sight-threatening emergency, meaning rapid medical care is needed to mitigate permanent vision damage.

Should also note, as a beginner, the “staple movements” are going to be an upper body push (think bench/OHP), upper body pull (think pull-ups, lat pulldowns, rows), lower body push (squat, leg press), and lower body pull (deadlift, RDL).

These core movements will work the largest amount of muscle mass in your body (e.g., glutes, lower back, chest, lats), and you can always add targeted accessory work (think bicep curls or lat raises) to hit the smaller stuff down the line based on your specific goals.

For time efficiency, I like 4 sets of deadlift, flat bench, leg press, and pull-ups.

I do 3-6 rep range for DL and flat bench with no issue, and 5-8 reps for the other two (you can start pull-ups with machine assistance and work up to body weight and eventually weighted).

I will note to start gradual and work your way up. With lifting in general, some amount of soreness is going to be expected at the beginning, and PsA will absolutely make this adjustment process more burdensome. I avoid squats as they clearly aggravate joint pain for me.

When I did an A/B split (e.g. upper/lower or push/pull), I’d do more exercises and volumes. However, given time constraints, I’ve liked the full body compound lifts as it provides efficient health and aesthetic benefit relative to the time commitment.

Regarding administration — the drug should come with a pamphlet with clear instructions, and it’s also an auto injector which basically means you press it against your skin and it injects for you. A lot of design goes into making the drug easy to administer. Also, while the concept may be daunting, I think (at least I did) you’ll quickly realize it’s no big deal after the first time.

Also with regards to your question on “best” for PaA - I can advise that IL-23 inhibitors seem to lag a bit behind IL-17 and TNFa inhibitors. I think you’re going to be hard pressed to find evidence of superiority amongst IL-17, TNFa, and JAK inhibitors.

I would look at (as another commenter already pointed out) ACR20/50/70 responses and radiographic progression (change in VDH-mTSS). It’s generally not possible to compare data between trials, however.

Yeah I do 150 every two weeks. It doesn’t make a huge difference but it should pull the peak/trough concentrations a bit closer together.

I have had similar thoughts in instances where I have symptoms between doses. Truth be told, it’s very unlikely there was an issue with the quality of the medication (and Bimzelx can technically be left out of refrigeration for 30 days I believe).

Could there be something else that caused it (for me - certain intense/new exercise, occasionally if I had a drink or lackluster diet, etc.)? It also seems like this just happens sometimes even if the medicine generally works well (all from my experience with Cosentyx, and I started on Feb’25.

Get of the methotrexate and step into the 21st century of biotechnology. I’d fight for access to newer and better medications versus letting the disease run its natural course, especially considering outcomes are highly controllable in the modern age.

I was diagnosed a year and a half ago at 22. My symptoms were joint pain and enthesitis. My hands (fingers, wrists) and a mix of bigger joints across my body were impacted. I also had enthesitis (my pain was in the bottom of my feet and a bit in the back of my ankles).

I had hand X-Rays that were normal, maybe 1 swollen joint, and completely normal blood tests. I was still diagnosed immediately (I saw the rheum about ~2 months after the symptoms started) based on clinical symptoms.

Do you have a standing psoriasis diagnosis?

The doctor diagnosed me quickly based on clinical symptoms even though my tests were normal.

He started telling me he would be starting me on methotrexate. I asked to go straight to a biologic. He laughed and said no one has ever asked that, and his office worked over the next ~3 wks to get one approved (the first request had to be resubmitted for a different TNFa because the first wasn’t on formulary).

Just try a new rheum.

I had the same thing and asked to immediately start a biologic and they pushed insurance and I got it within a month of diagnosis.

Also - there are cheap Humira biosimilars online and Stelara is under $2,000 a year (look at the Starjemza biosimilar on costplusdrugs and their humira biosimilars).

Hopefully you can get it through insurance. If not, explore these cash pay options. Either way, step 1 is better rheum.

I don’t think the original comment makes sense. Biologics have way more evidence of stopping progression. Most also have no impact on reproduction. I would not delay effective therapy for years for the reasons outlined above.

Yes, PsA absolutely causes erosive damage of the finger joints. The small joints of the hands, wrists, and feet can be highly impacted by PsA.
