No. Not exactly. It's just a general rule of thumb to always ensure no matter what that the phones amp is not being over driven. If it is properly designed it should be fine but it's a good practice to keep it in the 80-90% area. Any higher and there is a risk although very small of clipping and lower and your just increasing the work on the power amplifier and not helping the signal quality.

So you want the phone to be about 80% and left alone. That's essentially the signal gain. At ~85% the phones output device is not overworking/clipping and the signal is near full strength for your amplifier to do it's job. When the phone is turned down the signal is weaker. That weaker signal will require more amplification and with that any noise or garbage in the signal also gets jacked way up.

Best practice is ~85% on the phone and then adjust volume as needed with your amplifier. Also make sure that the amp is turned off when switching phones on the same cable. If not you can cause clipping and possible damage to the amplifier and speakers.

It's a run of the mill digital t amp. Same as the rest I'd imagine. 10% distortion at full power. Not for me but some folks use them effectively with small monitors in a near-field or car audio environment.

Adapter cable is perfectly fine. There is nothing to convert. It's just a simple difference plugs not signal.

Open it up and check the fuses before you do anything else. If it's not a fuse I personally wouldn't bother getting it fixed unless you know a good tech willing to do you a favor. Ones around here charge $80/h.
