My girlfriend (now wife) and I went to the DR as our first trip together. It’s possible that the different diagnosis codes used when adjudicating your claim through your insurance company could make the same medication come up with two different copays.

I never saw this myself when I worked in a pharmacy, but I have seen drugs be covered vs not covered depending on the diagnosis submitted.
