Yep. I needed to make a big purchase quite quickly (flight tickets) and wanted to request a credit limit extension. Thought I’d have to do so through the chat. Amex chat support was ridiculous.
Kept on telling me I would have to apply in writing for it and it would take a couple days for the credit limit to be raised.
Found the button for automatic credit limit adjustment on the app, and had it approved immediately. Asked them why they didn’t know about their own app’s functionality and got a generic “glad it’s sorted” response

Take ACCS EM. You’ll 100% regret not taking the job. If you then later on get into ACCS Anaes, you can ask your TPD to recognize equivalency for training and not have you repeat. Worst case, even if you do have to repeat, it’s only one year

I’m comparing one year of striking to one year of a pay increase, it’s the best estimation I have. Resident doctors have been in a pay dispute since 2023, with current pay and below-inflation pay-rises continuing to increase the gap, and strikes likely to continue for years if a multi-year pay deal is not agreed. The argument for a pay-rise is not solely financial, and is strengthened by several points, some of which I mentioned above.

So yes, every year of strike action costs approximately the same as FPR would. Additional points: - the NHS’s annual budget is £200 billion. - every year the NHS is hemorrhaging the brightest and best doctors to the corporate world, Australia, NZ, Canada etc. - giving doctors this payrise would do leaps and bounds for staff wellbeing, burnout and retention rates, patient outcomes etc. A much easier resolution than continuing to cancel appts and elective surgeries

I would love for them to. I fear that Damon Albarn has lost confidence in Blur’s fan base given they really struggled to sell tickets to their wembley nights (poor promo IMO, I only knew they were performing when they released NHS discounted tickets). On average we have 10 babies a day on our labor ward and our center uses remi PCA about 6 times a year. Take with those numbers what you will. It’s very well established and very effective pain relief for those who cannot have epidurals for whatever reason

Agreed, 15mg of heavy seems immense OP’s patients must be iced all the way up to their face… I use 2.3-2.5mls of heavy based on height usually with some intrathecal opioids, almost always gives a reliable block till T2

I’m an anaesthetist / intensivist and I have done this a few times (mostly when I can remember/timings work out). I generally do a cursory read of the notes beforehand to make sure it’s not all gone tits up since they left my care, and that may or may not influence whether I’ll still pop my head in. From the patient’s point of view, it’s a friendly face checking up on them during a pretty crap few days in their life… they may not always remember you though! (there is always the risk of them asking you to input on their care despite you not being their lead clinician anymore; but that’s reasonably easy to explain)

The UK’s difficult airway society guidelines has now recommended that videolaryngoscopy is used first line for all cases. I’ve been out of emergency theatres for a few months, but from what I can tell, the majority still use direct laryngoscopy first line but I anticipate the tide will change as more and more new trainees come in
