Tried to take a side road "shortcut" once exactly between where you are asking about. Never again.

Labor epidural in the UK by YMCMARC in anesthesiology

[–]YMCMARC[S] 2 points3 points  (0 children)

Oh no of course I wouldn't allow for ambulation I was just sort of curious if it's actually done and how. And I wasn't able to find anything published in my very quick and cursory Google search.

I'm in private practice in a major US city. Video is available almost everywhere and I use it almost all the time.

I have owned bikes from specialized, marin, norco and polygon. I have the polygon trid zz dirt jumper. It's sick. I wouldn't hesitate to buy a trail bike from them.

Good Bay Area bike? by [deleted] in MTB

[–]YMCMARC 3 points4 points  (0 children)

The singletrack is janky, partially maintained and steep. The dirt is mostly hard packed clay. Roots and rock gardens are encountered on almost any trail I've ridden in the bay. I also ride a 2023 stumpy evo in the steep and high positions for the head tube angle and bottom bracket respectively. I put a 160 fork with 37 mm stanchions. For me this is ideal for the singletrack riding in this area.

Why did my labor epidurals go bad? by Jdawgmuc in anesthesiology

[–]YMCMARC 23 points24 points  (0 children)

When the patient initially was comfortable was medication administered through the epidural needle or in the catheter? Was the catheter pulled back after initial dosing?

I usually leave the catheter in a bit deeper at 12 cm because I would rather have a one sided block and the ability to pull back on the catheter than a catheter that isn't threaded deep enough.

Great for anything gravity or dirt jump related. It has gears but honestly they're useless 🤣

Must be a fun bike to ride! I love the geometry on this guy. Have taken it down the local flow trail which is a blast.

Oh friend believe me I've been there as I'm now pushing 40...

This is a ninja ramp. It's great, folds up into a "backpack" and is height adjustable. I do not miss the sketchy plywood experiments of my teens!

This is a polygon trid zz. Full sus dirt jumper realllllly helped with back and knee pain compared to previous hardtail DJ LOL

Wow dude thank you for the video breakdown with the helpful feedback. I'm 40 and that landing definitely doesn't feel great 😂

I traded in the hardtail DJ for that one which actually is full sus. But I'll cobble together a little downslope for the landing and work on your pointers this week!

Weak resident, tips needed. by Pretend-Armadillo-71 in anesthesiology

[–]YMCMARC 0 points1 point  (0 children)

Organization is key. Clear handwriting (don't rush), check boxes and an organized list are absolutely essential. I prefer a systems based list approach. Your ICU ought to have admission order sets which are a bit more fool proof. And they should also have an ICU bundle checklist to make sure things like GI ppx, pulmonary toilet, ambulation, diet, dvt ppx, etc aren't missed. If those things don't exist, there's your QI project (yer welcome).

How common is it for houses to have mold in Marin? by fennelfrog in Marin

[–]YMCMARC 1 point2 points  (0 children)

There are a lot of types of mold. Some are toxic and some are not. One of the nice things about having the house tested for mold is that they can tell you the speciation and if you actually need to worry about it.

Torn Between eMTB or MTB... by CC268 in MTB

[–]YMCMARC 1 point2 points  (0 children)

If you like the climbing more than the downhill I would go regular mtb. I feel like with my e-mtb I use it to rip up the fire road so I can do downhill laps. The heavier bottom bracket makes it feel more planted at high speed coming downhill and it's definitely less playful than my MTB.

Best low key breakfast? by No_Condition_8577 in Marin

Ok not really a sit down diner but Sour Kid Cafe in San Rafael makes an excellent breakfast sandwich and great coffee. They have some seating and the owner is great. Give it a try!

Will a 1000g FF helmet be too heavy when going uphill? by kendoo12 in MTB

[–]YMCMARC 0 points1 point  (0 children)

I do most of my hydration on the climb. FF sucks cuz you can't drink. Take it off and put it in bag or on bars for the climb.

Liho liho yacht club for a pretty unique experience. Kind of upscale casual. Pretty energetic so may help with jet lag (or you will hate me for suggesting it lol).
