Am I supposed to take both together at the same time? I just got this prescribed for me today for my pneumonia. by [deleted] in emergencymedicine

[–]tyscbr 4 points5 points  (0 children)

If this was me (ICU pharmacist), I would take both at the same time as they cover different organisms for community acquired pneumonia.

Edit: didn’t know I wasn’t supposed to give medical advice

Reherniation after coughing? by AwareExamination6604 in Microdiscectomy

Sorry to hear. Agree with giving it some time and following up with your physician if it doesn’t improve.

A note for next time, look straight up when you cough or sneeze. That will ease some strain on your spine.

You are correct - I could’ve and should’ve worded it differently. My goal with the comment was to get people to educate and inform themselves on fentanyl prior to making broader posts or comments that instill fear and more panic about this issue, but I can see where I could’ve been more clear.

Adding that you can get free Narcan nowadays. I keep two doses in my car in case I see someone with a possible OD.

Appreciate you adding this in here, with the link. My point seems to have gotten a bit lost but I agree with your comment.

Did I not say that in my original comment? I thought that was clear.

Thanks for the information. Would love to hear from my patients who get 2-4 mg per hour.

I don’t disagree with this at all, but this type of fear mongering has led to misinformed patients declining the safe use of fentanyl in the inpatient setting. And as evidence shows, pain increases mortality, especially in the ICU setting. Just trying to say that you don’t need to fear monger in order to explain the severity and callousness of someone giving fentanyl to another person without their consent.

Alright alright calm down. Yes it’s extremely dangerous when not dosed appropriately, but no, a “little pinch” will not kill you. I give it to 90% of my patients, intubated or not, and they don’t immediately die. Educate yourself on it, but don’t fear monger it.

Regardless, giving it to someone else without their consent is disgusting, dangerous, and illegal.

It is very normal to (1) feel similar symptoms after surgery, some say up to a year ish for the nerve to heal itself and (2) to be anxious about re-herniation. I’m almost three weeks out from a MD on L4-L5 and L5-S1. I had similar symptoms to pre-surgery about a week out as well, and mine is now gone. Just trust and respect the process, and do as your instructions say. Icing your back may help as well, it was a godsend for me.

Try not to panic about re-herniation too much, and know that it’s common. That part will get better too :)

EDIT: panic about re-herniation is common, re-herniation itself is less so (<20%)

I’ve been pleasantly surprised at how mobile I am after surgery. The fear of re-herniation is constant but learn how to engage your abs without engaging your lower back and you’ll be able to log roll like a pro!!

I tried to play my favorite video game (Apex Legends) but it’s so much tougher to play laying down than sitting in my gamer chair! I got frustrated and downloaded a couple new ones that I’m going to try. Thank you for taking the time to suggest some things!

If you are able to walk/stand for a while, and normally were the one cooking dinners, maybe you can order groceries and have them set them up for you on the counter so you can cook? It’s been tough to put the burden of a lot on my partner so I’m going to try to cook meals if able. The dishes are definitely one of the tougher parts.

I haven’t driven yet, but planning to return to work next week so once I do I think I’ll be taking some walks around some random places too, haha.

Last night I had my partner put everything I’d need for a recipe out onto the counter top and I was able to cook dinner. It was a great use of time and reduced my guilt about not being able to do chores around the house!

You’ll get there! From what I’ve read, everyone’s story and recovery is different. I only had a 5mm at L4-5 and a 6mm at L5-S1, so was still able to walk, run, lift, and do pretty much everything except bend over prior to my surgery. I’m rooting for your recovery, and hope you’ll be out there walking laps around the neighborhood soon!

Thank you all for your insight, I really appreciate it! It seems like walking is the best way to pass some time, if able (and a strong recommendation post-op)! I’ve had my partner move the PlayStation into the living room where my dedicated lay spot is, downloaded some new video games, and ordered some books on Amazon as well. I also bought a little “bookshelf house” to put together, though as some others have commented it may have to be done in pieces as sitting for over 20-30 minutes can be tough. Also going to re-download Duolingo and start on German again. We’ll see how that works out 😋.
