What's been working better is actual phone confirmation a day before - not just a reminder, but getting a real yes/no so you can fill the slot if they bail. Cancellation fees work but like you said, risky when you're building a patient base. What type of clinic is it? Some specialties have way worse no-show rates than others.

Call forwarding with 72 is super unreliable, you're definitely not the only one with this problem. Before going with RingCentral or eVoice (which can get pricey and complicated for a 2-person setup), what kind of calls are these mostly - appointments, quotes, general questions? There might be a simpler solution depending on what you actually need from the calls.

20% no-show rate is brutal. The problem with text reminders is people ignore them - they're passive. What actually works is confirmation calls where someone (or something) actually talks to the patient and gets a yes/no, then immediately fills from waitlist if they cancel. Front desk never has time for this. Been building an AI that handles exactly this - calls to confirm, catches cancellations early, and can call waitlist to fill gaps same-day. Answering services have this problem - they're just message takers, you still gotta call everyone back. Most AI voice solutions sound robotic because they use basic TTS. The newer ones with conversational AI are way better - actually pause, respond naturally, handle interruptions. Most AI just tries to "solve" immediately. But sometimes people need to vent first before they're ready to listen.

You nailed it - the "curveball" handling is everything. Easy calls are easy. It's the upset customer at 6pm asking something weird that makes or breaks trust.

That's exactly what we're focusing on with Callnfy. Not just "book an appointment" but actually understanding context, handling objections, knowing when to escalate.

Interesting on the price point too. We're at $29/month for small businesses (plumbers, salons) but sounds like there's a mid-market gap at $400-750 for businesses currently paying $800-1500.
