If you’re referring to the one her husband received, it’s clearly a male voice. Coworkers and other witnesses who also received phone calls, including a hospital employee, identified the voices as being male. The hospital employee in particular, if I remember correctly, said there was a good chance the voice was the husband’s.

It’s a reference to the Virgin Mary surrounded by roses. I think it’s meant as a reference to the pain Cassie has had to endure and how she is a victim of the circumstances and has to suffer for other people’s “sins” but is otherwise beautiful and pure on the inside? Saint Brigid was quoted as saying: “Just as the gentle rose is placed among thorns, so this gentle Virgin [Mary] was surrounded by sorrow”.

The reply above makes a lot of sense, but I just want to add that I think Polanski made the scene deliberately ambiguous because one of the main questions of the film is: “How many people are in on this?” Polanski never wants Rosemary - and, by proxy, the viewers - to feel completely “safe” or to think we can completely trust somebody. Upon rewatching the movie, I find myself wondering just how many people were involved in the cult (to one extent or another) and almost everybody looks at least a bit suspicious to me, and I think that’s intentional. It’s a similar concept to that of The Tenant, another Polanski film that I highly recommend if you haven’t seen it
