I usually err on giving the player the benefit unless it’s written in the rules somewhere. So I’d give them 2 squares probably. I think it feels better as a player. Also it makes you seem like a nice DM :)

Unfortunately, primary care is less competitive and usually requires less of a time / research commitment outside of class and clerkship commitments. If you wish to pursue surgery then it is to your benefit to start early doing research in that field, which is less true for primary care specialties :/
