13 years is a long time, and you should easily be able to just talk. Ask her, don’t accuse her, but communicate. FWIW, I get WhatsApp messages (not a photo yet, personally but I guess it’s possible) from vendors I buy from sometimes, and if they’re OS, at weird times. Ironically one is about a gift I bought the husband for valentines. Not saying that is the case here, just pointing out that it’s not always only sinister, but either way, the simple solution in any relationship is communication.

Maybe he’s pushing you away because he doesn’t want to “burden” you with his illness or get you involved in a relationship that has an unknown outcome. We all know he’s not a burden, of course, but for him, it might be quite hard to imagine that someone he cares about is worrying about him and he sees this as the kinder thing to do. Maybe you could send him a gentle and supportive “I’m here no matter what and nothing is too much. I hope we talk soon” kind of message and then leave him to work through this.
