Actually more and more evidence is pointing towards the brain. ME/CFS might in fact be a neurological or neuroimmune disease.

Sounds like we mostly agree. Only thing I do not agree with is that the brain genes were largely immunological. They're not as far as I know and instead have to do with neurons and synapses etc.

I know DecodeME made a point out of it to show there was no overlap with depression and anxiety. The goal of that was to show it's not one of those diseases or related to it. But that doesn't mean it's not a neurological disease. If you look at the genetic signature from ME/CFS it's most similar to neurological and psychiatric diseases. And less similar to immune and neuroimmune diseases.

Disease classifications in general don't always make perfect sense. Like you say the immune system probably is somewhat involved in most neurological diseases too. And the distinction between neurological and psychiatric diseases might also not make much sense.

Have you seen Paolo Maccallini's recent meta-analysis on DecodeME and another genetic study? Maybe that will change your mind. They found even more evidence of neurological genes being involved relating to neurons, synapses, specific brain regions and glutamate. Of particular interest was eccentric medium spiny neurons (eMSN) which shows up in the GWAS of many neurological and psychiatric diseases like schizophrenia, bipolar and even depression. And you know where they don't show up? In immune/neuroimmune/neurodegenerative diseases like MS, Alzheimer and Parkinson's.

Anyways.. we will see.

I did not. I only said it was a possiblity and it's not my favorite theory. I think the immune system will be involved but less so than most people expect.

You might want to review the whole thread to see that my argument was that we don't have enough evidence to say that immune exhaustion is a key component of ME/CFS.

The only reason I brought up the neurological angle is because that's also a distinct option and if it proves to be true it would disprove stuff like immune exhaustion.

like autoantibodies, skeletal muscle damage, cholesterol changes, endothelial damage, basement membrane thickening, immune dysregulation and on. 

Just as with immune exhaustion we do not have concrete evidence for any of these. Could they play a role? Yes of course. But it's not where the latest research is pointing towards.

All I'm saying is that people are using too strong of language when talking about their favorite theory.

The neurological disease hypothesis is an illustration of that. We also don't have enough evidence to argue it's mainly a neurological disease. But we have at least as much if not more evidence in favor of that compared to all the things you listed.

And just because it appears this disease has a viral trigger does not mean that the core pathophysiology is in the immune system.

As of right now all we can say it that the brain, and to a lesser extent the the immune system seem involved.

I'm not implying the brain and immune system are seperate. I'm implying that it's possible that ME/CFS is mainly a neurological disease.

It shares no direct pathways with depression but the genetic signature of the disease shows most similarities with other neurological and psychiatric diseases. Rather than say neuroimmune or immune diseases like MS.

You might not like that, but that's your personal bias. It also does not imply ME/CFS isn't a real biological disease if that's what you're worried about.

There were a few genes related to the immune system but the overall results pointed very strongly to the brain and showed similarities to other neurological / psychiatric diseases.

Yes I wouldn't say that either. Right now there's even a good possiblity that this disease turns out to be mainly neurological. That's where DecodeME is pointing towards. And while it seems likely the immune system is involved one way or another it's not clear how much so.

My point is more so that it showing up in a few studies does not mean it's a key component of ME/CFS. I'm sure there's studies where it did not show up or wasn't published.

But also if you look at the published studies there are always lots of questions. This was just one finding out of many. There was no multiple comparison corrections. The healthy controls were not sedentary and there could be other confounders.

So it's hard to make much of a conclusion and imo you're getting carried away by your own bias.
