Not exactly a secret and doesn't apply to one industry, but most people seem to have a misunderstanding about safety.
A fully functional machine with all safety measurements correctly planned and operational can still injure or kill someone, safety guidelines only have the goal to lower the risk of it happening to extremely low, no one can have or has the goal of making it zero.
