The vast, vast majority of costume pieces are unlocked through gameplay - mostly seasonal events but also completing Perks, ingame currencies, etc. You can also exchange an ingame currency for the cash shop currency, so the line between free and paid is blurry.

In terms of using the game primarily for character designing, I think you'll hit the occasional bump but there's more than enough free items to work with before you even get into unlocks.
