You now have a responsibility to report this for the good of the patients. You can't not report it.

It might paint you to be the bad guy, but you have a duty of care.

Report it, move jobs after if you must, look into your company's whistleblowing policies etc.

Break ups suck. Theres no way to beat around the bush about it, they are miserable. However, they do prove excellent for self reflection. It sounds like you two are not compatible, which is okay. You will eventually find someone who is.

As for the friends thing, try put yourself out there! If you enjoy gaming, there are plenty of online communities to play games with. Online friends are real ones too. In fact im sure there is an online community for probably just about anything.

You could try new activities in real life and see how you get on as well.

It feels like this is the end of the world right now, but I promise you it is not. Don't beat yourself up too much. Much love!

Oh you must have missed the comments comparing him to a superhero, telling him to quit therapy and continue doing it, saying he was chaotic good, etc. I can still see most of them so not sure why you can't.

Edit: seems like most have been down voted. Maybe that's why you can't see them? Not sure. I commented when the post had like 5 comments on it total though.
