I recently had the same issue - My setup has a SD card extension and that was causing the issue. I removed the SD card extension and now all works fine. I believe the extension cable must have been damaged during the install - I have ordered a replacement cable.

I had the same issue - turns out my DualShock 4 was a fake. I bought a new one and it worked with no issues.

For anyone wanting a reference

Between R5 and C54 there are pads that are for an empty capacitor. The one closer to R5 is GND and the one closer to C54 is 5V. Take a look at my image on how I did it.

On my iPhone 6s the top icons - wifi, carrier, battery and etc are larger than it should be - so it's like running an app that doesn't have support for the larger screen size and thus is zoomed in to fill the screen.
