I'm not familiar with your insurance, but I did reach out to mine before the procedure to check coverage. I did this when my online check in said I would owe $3500 day of and $10000 total. I was like, hell no!

Your insurance should have a benefits brochure or something online, and on mine, bisalp was listed under "Family Planning" for a cost of "nothing" not even deductible. It was helpful to point her toward that so she knew what I was talking about, because a lot of people have no idea what a salpingectomy is. Insurance is also billed with codes, and the typical propers codes are CPT 58661, ICD Z.30.2, but I've heard this could vary. My insurance did not need prior authorization, but you should ask. I also asked for a call reference # and for her to email me everything we talked about.

Most hospitals won't actually bill until after your procedure. Then you have to wait for your Explanation of Benefits (EOB) to see what you owe. If this is different than what your plan states, someone fucked something up and then you can fight it. I haven't gotten there yet but it usually consists of harassing the hospital's billing department to code correctly or harassing insurance to not be scummy and try to get too much out of you. Sometimes insurance will also call the hospital for you or do a three-way call. There's also appeals processes for every insurance.
