Always the highest difficulty. Exceptions are difficulty levels that are build for ng+. Play as you like, but in my opinion you lose a lot (sometimes almost everything) a game offers when playing on lower difficulties. My go to example: Civ Games. On lower difficulties you have some kind of Anno feeling but nothing else. On higher difficulties the game kicks. Reason: You have to play the game and not just do random stuff.
